• Impression

• Opposing Model

• Bite registration & Stick Bite

• Study Model (Pre-op & Temp)

• Final Shade

• Photo

1. Download a digital Lab Request Form from here.

2. Place case materials in sturdy shipping box. If you don’t have one we will send our shipping box and wrapping materials so please make a request at 301-525-3974 or kyoridentallab@gmail.com. Make sure to use the bubble wrap or any other wrapping materials in the box and nothing is rattling as this will ensure your models and other case materials are not damaging during shipping. If you are sending STL file, simply send it to kyoridentallab@gmail.com. 

3. Call us for local pick up or send case via delivery services that is traceable to the following address.
